Program Description: Grief can be confusing, lonely, and full of ups and downs. In this program, we’ll talk openly about what grief can feel like, what’s normal, and what can sometimes make it harder. Together, we’ll share ways to cope, find comfort, and support one another through loss—because no one should have to navigate grief alone.
Sara Chambers BSN, RN, CHPN is the Community Educator, a Certified Advance Care Planning Facilitator, and Nurse Planner for Hospice of Northwest Ohio. With seventeen years as a nurse, mostly in critical care and hospice, she feels honored to have found her niche in nursing. Sara is proud to uphold a national certification in Hospice and Palliative Care. She passionately believes in advocating for patients and their families through education related to: the Hospice philosophy, advance care planning, death and dying, and symptom management at the end of life.
Some of Sara’s responsibilities at Hospice of Northwest Ohio include coordinating requests for outreach opportunities for the community and assisting with developing and/or presenting any new educational offerings. She also provides free Advance Care Planning to anyone in the community who would like assistance in completing Advance Directives and takes part in internal education.
